Case Manager

West Central Arkansas Planning And Development District, Inc is seeking a skilled Case Manager to join our team. As a Case Manager, you will play a critical role in coordinating services for adults with developmental disabilities, ensuring compliance with DDS, Medicaid, and DOL standards in a clinical day treatment setting.
Key Responsibilities:- Coordinate services for new admissions, including assessments, goal/objectives, prescription tracking, staffing/progress reports/data/admission/discharge
- Assess referrals, determine eligibility, and obtain necessary documents
- Maintain consumer files per DDS/Medicaid standards and complete mandatory reporting
- Conduct Career Counseling per WIOA guidelines
- Adhere to DOL guidelines Sec, 14c
- Comprehend and apply DHS, DDS, Medicaid, DOL, and WIOA standards
- Assist with behavior issues, needs, and crisis intervention
- Apply positive behavior practices
- Communicate with families, caregivers, PASSES, and agencies
- Coordinate and plan events, including volunteer recruitment and speaker engagement
- Work closely with other departments as necessary
- Conduct tours and assist with break duty/classes as needed
- Complete required annual training as directed by DDS, AUI, and other regulatory agencies
- Practice HIPAA/privacy practices and handle sensitive and confidential information
- Review weekly client pay run for accuracy
- Know community resources or be able to gain this information
- Bachelor's degree in Human Services, Social Work, Counseling, or a relevant field
- 1-3 years' experience working with persons with disabilities preferred
