Job Summary

We are seeking a skilled and compassionate RN to join our Hospice team. As a Hospice Nurse, you will provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan. You will perform services in accordance with the physician's orders and the established plan of care (POC), ensuring delivery of care is compliant with federal and state regulations.

Responsibilities

Qualifications
  • Must be a graduate of an approved school of clinical education
  • Must be licensed in the state where they currently practice
  • Must have at least one year experience as a licensed professional
  • Must have basic demonstrated technology skills, including operation of a mobile device
Requirements
  • Must possess a valid state driver license
  • Must maintain automobile liability insurance as required by law
  • Must maintain dependable transportation in good working condition
  • Must be able to safely drive an automobile in all types of weather conditions
  • Must possess CPR certification for the healthcare provider
