Pinnacle Home Care Inc. - Physical Therapist Job Description

We are seeking a dedicated and compassionate Physical Therapist to join our team of skilled healthcare professionals at Pinnacle Home Care Inc. in Putnam County.

Job Summary:

The Physical Therapist will provide high-quality Physical Therapy services to patients in their homes, working closely with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities:

  • Conduct comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
  • Develop and implement treatment plans in consultation with physicians and revise as necessary.
  • Collaborate with the interdisciplinary team to develop and implement client care plans.
  • Teach, supervise, and counsel patients and their families in physical therapy programs.
  • Supervise Physical Therapist Assistants at least every 30 days.
  • Communicate with physicians and other team members regarding patient status and obtain additional orders as needed.
  • Write clinical/progress notes on the date of the visit and incorporate same in the client's record according to company policy.

Qualifications:

  • Graduate of an accredited Physical Therapy program or equivalent education.
  • Licensed to practice as a PT in the State of Florida.
  • Current BLS Certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
  • At least one year of experience as a Physical Therapist.
  • Home healthcare experience is preferred but not essential.
  • Electronic medical record experience is preferred but not essential.
  • Must display good emotional health and be able to physically tolerate much standing, stooping, bending, and heavy lifting; must meet all agency regulations.
  • Must be self-directed and able to work with little supervision and have good observational, judgment, and communication skills.
