Pinnacle Home Care - A Leader in Home Health Care

We are seeking a dedicated and compassionate Physical Therapist to join our Putnam County team. As a Physical Therapist with Pinnacle Home Care, you will have the opportunity to make a difference in the lives of our patients and their families.

Responsibilities:
  • Provide comprehensive assessments of patients' needs, including OASIS assessments.
  • Establish and revise treatment plans in consultation with physicians.
  • Develop and implement interdisciplinary client care plans as it pertains to Physical Therapy.
  • Teach, supervise, and counsel patients and their families in physical therapy programs.
  • Communicate with physicians and other healthcare professionals to ensure seamless care.
Requirements:
  • Graduate of an accredited Physical Therapy program.
  • Licensed to practice as a PT in the State of Florida.
  • Current BLS Certification.
  • At least one year of experience as a Physical Therapist.
