Primary Care Nurse Practitioner and Physician Assistant Opportunity in Connecticut

We are seeking a skilled Nurse Practitioner (NP) and/or Physician Assistant (PA) to join our esteemed Primary Care team in New Haven County, Connecticut. As a valued member of our healthcare organization, you will have the opportunity to provide high-quality patient care and make a meaningful impact in the lives of our patients.
Job Summary- Conduct comprehensive medical examinations, obtain thorough medical histories, and provide treatment for acute and chronic illnesses.
- Order, perform, and interpret diagnostic tests to inform patient care.
- Develop personalized treatment plans, engaging patients and family members in the decision-making process.
- Initiate referrals to specialty services as needed.
- Collaborate with management, physicians, and staff to improve care delivery and service quality.
The ideal candidate will possess at least 1 year of experience in Primary Care, graduation from an accredited APRN or PA program, and current licensure as a CT Nurse Practitioner (NP) and/or Physician Assistant (PA). A strong understanding of clinical assessment, documentation skills, and interpersonal communication is also essential.
