About the Role

We are seeking a highly skilled Nurse Practitioner to join our team at Abode Care Partners. As a Senior Medical Care Professional, you will be responsible for providing evidence-based medical care to seniors and patients with complex needs in various settings.

Responsibilities
  • Perform comprehensive medical assessments, develop, and implement individualized plans of care
  • Collaborate with healthcare professionals to provide interdisciplinary care and ensure seamless transitions
  • Order and interpret laboratory and diagnostic testing as needed
  • Provide referrals to other healthcare practitioners when necessary
  • Ensure patient medical records are up-to-date and accurate
Qualifications
  • Active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
