Key Responsibilities:
- Provide comprehensive primary care services to patients, including routine check-ups, health screenings, and disease management.
- Work collaboratively with a multidisciplinary team to coordinate patient care and achieve optimal health outcomes.
- Develop and implement individualized care plans that address patients' unique needs and health goals.
- Stay up-to-date with the latest clinical guidelines and best practices in primary care, and apply this knowledge to improve patient outcomes.
- Participate in quality improvement initiatives and contribute to the development of new clinical programs and services.
Requirements:
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA).
- Current licensure through the Florida Board of Nurse Examiners or Licensed Physician Assistant by the Florida State Medical Board.
- Valid DEA license or ability to obtain prior to employment.
- At least 1 year of clinical experience as a Nurse Practitioner or Physician Assistant.
- Comfortable managing a primarily Medicare-aged patient population.
Preferred Qualifications:
- Bilingual (English/Spanish) fluency.
- A working understanding and support of a managed care environment.
