Job Purpose

As a Prevention Specialist at Aspire Health Partners, you will play a vital role in assisting participants in optimal daily living by providing essential functions, including:

  • Linking participants with resources and services.
  • Delivering individual and group training on life skills.
  • Documenting participant information in the Electronic Health Record.
  • Following program-specific protocols and maintaining required credentials.
Responsibilities

As a Prevention Specialist, your key responsibilities will include:

  • Providing resource linkage and connection to program participants.
  • Delivering individual and group training on life skills to participants.
  • Documenting clearly and effectively in the Electronic Health Record.
  • Following all program-specific protocols in relation to specified role.
  • Maintaining all required credentials necessary to perform job functions.
Qualifications

To be successful in this role, you will need:

  • A High School Diploma.
  • At least 1 year of experience working with the target population.
  • A Level II Background clearance.
  • A Motor Vehicle Records clearance.
