Job Summary

We are seeking a skilled Care Coordinator to join our team at WVU Medicine. As a key member of our healthcare team, you will play a vital role in ensuring seamless transitions for our patients, from hospital to home or other care settings.

Key Responsibilities
  • Develop and implement comprehensive transition plans for patients, taking into account their unique needs and circumstances.
  • Collaborate with multidisciplinary teams to ensure timely and effective discharge planning, minimizing delays and readmissions.
  • Provide education and support to patients, families, and healthcare providers to promote smooth transitions and optimal care outcomes.
  • Monitor patient progress, intervening as necessary to address any challenges or concerns.
  • Facilitate referrals to post-acute services, including home care, durable medical equipment, and skilled nursing facilities.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless transitions and high-quality care.
Requirements
  • Current licensure as a Registered Professional Nurse in West Virginia or a temporary practice permit.
  • Five years of clinical experience, with a focus on transition and discharge planning.
  • Strong knowledge of patient insurance coverage, precertification requirements, and managed care principles.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
Preferred Qualifications
  • Bachelor's degree in Nursing (BSN)
  • Experience with electronic health records and quality screens.
Working Environment

This role requires frequent walking, standing, and lifting, as well as the ability to work in a fast-paced environment with multiple priorities.
