As a Home Health Occupational Therapist, you will play a vital role in helping patients regain their independence and improve their quality of life. Your expertise will be instrumental in developing personalized rehabilitation plans that cater to each patient's unique needs and goals.

Key Responsibilities:
  • Conduct thorough assessments to identify patients' strengths and limitations, and develop targeted interventions to address their functional deficits.
  • Collaborate with the healthcare team to create comprehensive care plans that align with each patient's goals and preferences.
  • Provide one-on-one therapy sessions to patients, focusing on activities that promote independence, mobility, and overall well-being.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination and optimal outcomes.
  • Stay up-to-date with the latest research and best practices in occupational therapy, and incorporate evidence-based interventions into your practice.
Requirements:
  • Master's degree in Occupational Therapy from an accredited program.
  • Minimum of six months of experience in occupational therapy, with a focus on home health care.
  • Current licensure as an Occupational Therapist in the state of practice.
  • Current CPR certification and valid driver's license.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
