About the Role

We are seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will be responsible for assessing, planning, implementing, and evaluating the care of patients in their homes.

Key Responsibilities
  • Provide skilled intermittent care to patients in their homes, including instruction and education to promote maximal functional status.
  • Regularly evaluate and re-evaluate patient status to ensure optimal care.
  • Initiate, develop, implement, and revise the patient's plan of care (POC) to address all identified problems.
  • Communicate with other disciplines involved in the case as warranted and report signs/symptoms signaling a change in patient condition to the patient's physician and case manager as needed.
  • Reassess the patient within 48 hours following a change in patient condition or discharge from a 24-hour or longer hospital stay, or as ordered by the physician.
Requirements
  • Current PT licensure to practice Physical Therapy in Washington State.
  • Recent applicable clinical experience in a medical/surgical unit, rehab, or home health setting.
  • Current Washington State Driver's license and proof of insurability.
  • Current Healthcare Provider BLS certification by date of hire.
  • Reliable transportation.
Preferred Qualifications
  • Bachelor's, Master's, or Doctorate in Physical Therapy.
  • Home Health experience.
