The RN Case Manager will play a critical role in creating and maintaining a plan of care, serving as the coordinator for patient and family interaction with our organization. This position requires strong communication and interpersonal skills, as well as the ability to provide sensitive and empathetic care in emotionally intense situations.
Key Responsibilities:
- Develop and implement individualized care plans that meet the unique needs of each patient
- Coordinate with healthcare teams to ensure seamless care delivery
- Provide education and support to patients, families, and caregivers on disease management, palliative care, and end-of-life care
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Review and update patient records, including care plans, physician orders, and pharmacy profiles
Requirements:
- Graduate of an approved nursing program
- Current Florida RN license
- Minimum one year experience in an acute-care hospital environment providing direct patient care
- Previous experience as a hospice RN preferred
