Job Summary

The RN Case Manager provides direct care and oversees home health services for clients within the community. This role collaborates with the client, their family, and the healthcare team to develop and implement goal-centered care plans, considering quality measures and financial aspects of treatment.

Responsibilities
  • Provides skilled nursing care to clients and clinical oversight to professional and unlicensed personnel.
  • Builds and maintains respectful relationships with others.
  • Collaborates with physicians/non-physician practitioners and the home health team to develop and update the client's home health plan of care.
  • Serves as a client advocate, ensuring safety needs are addressed by assessing the home environment and developing individualized abuse prevention plans.
  • Meets documentation standards, ensuring accuracy, timeliness, and compliance with payer/billing requirements.
  • Provides education to the client, family, caregivers, licensed clinicians, and unlicensed personnel.
  • Coordinates client care with family, caregivers, community resources, and other healthcare members.
  • Provides clinical oversight, delegation, and leadership to licensed clinicians and unlicensed personnel.
  • Ensures effective and appropriate financial management of the client's home health services.
Requirements
  • Currently licensed as a Registered Nurse (RN) in the State of Minnesota.
  • Proof of negative TB testing.
  • Access to a dependable automobile for travel to multiple company business stops.
  • A valid driver's license and proof of current car insurance when using a personal vehicle for company business.
  • U.S. Citizenship or evidence of valid Alien Work Permit.
  • Disclose any conviction and criminal history records pertaining to any crime related to the provision of health services.
  • Pass initial and ongoing background studies and screenings.
Preferred Qualifications
  • At least two (2) years' experience in a healthcare facility or home care field.
  • Critical thinking skills, the ability to follow standards/procedures, and make appropriate clinical judgments to provide safe and effective client care.
  • Ability to work independently with minimal supervision, adapt to change, and manage a variety of assignments.
  • Ability to build and maintain professional relationships.
  • Excellent written and oral communication skills, interpersonal communication, and teaching skills.
  • Basic computer skills and willingness to learn new software programs.
