About the Role

Cassia is seeking a skilled Nurse Lead Transitions Liaison to join our team in Assisted Living. As a key member of our senior care team, you will play a vital role in ensuring seamless transitions for our residents.

Key Responsibilities
  • Manage Cassia AL locations open beds with the needs of referral sources.
  • Interact and establish trusted relationships with community partners and referral sources within a defined geographical region of Minnesota.
  • Understand assisted living, skilled nursing, hospice/home care, and adult day care clinical capabilities to ensure appropriate services are initiated on behalf of the individual.
  • Work proactively with social services/case management to find appropriate patient placement.
  • Collaborate with hospital social services/case management in patient/family communications, explaining benefits and care options to patients and their families.
  • Review patients' medical records and initiate the pre-admission plan of care and all standard admission activity to completion.
  • Coordinate with discharge location staff and Cassia AL locations, ensuring a smooth transition of care needs from one care environment to another.
  • Verify health insurance, complete required paperwork, manage correspondence, and maintain data in EMS; track referrals, admissions, and conversion ratios.
Requirements
  • A current nursing license (LPN or RN) or Social Worker license (LSW) with assisted living care experience within senior care, or an individual with 5+ years of Case Management or Skilled Care Admissions experience.
  • Ability to travel and work in various healthcare settings, including hospitals, clinics, assisted living centers, skilled nursing facilities, rehabilitation centers, and prospect homes.
  • Self-directed and self-motivated with exceptional verbal, written, active listener, and interpersonal skills.
  • Knowledge of electronic health records and excellent organizational skills, with the ability to effectively manage time and multitask.
