Position Overview

The Nurse Practitioner is expected to operate autonomously while also collaborating effectively with other healthcare professionals. This role involves consulting with management, physicians, and medical directors to ensure adherence to established guidelines. Participation in risk management, quality assurance programs, and clinical meetings may be required.


The Nurse Practitioner will uphold rigorous departmental objectives, performance standards, regulatory compliance, and quality patient care protocols. This role functions independently as a certified nurse practitioner, providing care to a designated patient population in partnership with a licensed physician. Practice is conducted in accordance with a formal practice agreement.



Key Responsibilities

PRIMARY DUTIES:

  • Conducts independent assessments of both acute and chronic clinical issues. Performs and documents comprehensive physical evaluations and patient histories, analyzes patient condition trends, and formulates, documents, and executes a management plan based on collected data. This includes contributing to the nursing care plan and providing essential counseling and education to patients and their families.
  • Develops patient care strategies grounded in thorough knowledge of the specific patient demographic and/or protocols, proactively identifying potential physiological and psychological challenges while considering the patient's cultural background, comprehension level, personality, and support systems. Acts as an advocate for patients.
  • Patient management encompasses the following:
  • Writes orders for admissions, transfers, and discharges.
  • Orders and interprets relevant laboratory and diagnostic tests.
  • Prescribes appropriate medications and treatments.
  • Refers patients for specialist consultations as necessary (e.g., dermatology, neurology, etc.).
  • Maintains detailed progress notes and summaries.
  • May perform certain invasive procedures independently upon demonstrating documented competency.
  • Engages in patient care rounds and discussions, effectively communicating management strategies to the healthcare team. Collaborates with multidisciplinary team members to ensure successful patient management.
  • Identifies situations requiring immediate physician intervention and initiates life-saving measures when necessary.
  • Utilizes advanced communication skills to navigate complex scenarios and enhance patient service delivery.

Additional Responsibilities:

  • Collaborates with multidisciplinary team members to assess and improve current healthcare delivery systems, implementing new practices as appropriate.
  • Engages in professional development activities that promote personal and professional growth.
  • Works in conjunction with physicians, nurses, and other healthcare professionals to facilitate patient transitions to lower levels of care when medically suitable.
  • Introduces self to patients and families, clarifying the role of the nurse practitioner.
  • Facilitates family conferences to discuss treatment objectives, optimize resource use, provide education, and identify post-discharge needs.
  • Fosters collaborative relationships to empower patients and families in making informed decisions regarding care goals, including palliative and hospice care.
  • Coordinates discharges to appropriate care levels, utilizing preferred providers for additional services as needed.
  • Prescribes medications in accordance with state regulations.
  • Performs other duties as assigned by management.

  • Basic computer proficiency and familiarity with Microsoft Office Suite.
  • This position may necessitate local travel.

Education and Experience Requirements:

  • Advanced degree in Nursing (ARNP or equivalent) required.
  • Current Nurse Practitioner Certification in the relevant state required.
  • Board certification by AANP or ANCC preferred, but may be mandatory in certain states.
  • Current DEA registration for controlled substances may be required based on state regulations.
  • A minimum of one year of clinical experience preferred, though strong new graduates may be considered.
  • Basic Life Support (BLS) certification from an accredited organization required within the first 90 days of employment.
