Advanced Practice Nurse

The Nurse Practitioner is expected to operate autonomously while also collaborating with a team of healthcare professionals. This role involves consulting with management, physicians, and medical directors to ensure adherence to established guidelines. Participation in risk management, quality assurance programs, and clinical meetings may be required.
The Nurse Practitioner will maintain compliance with departmental objectives, performance standards, regulatory requirements, and policies. This position functions independently as a certified nurse practitioner, managing a designated patient population in conjunction with a licensed physician.
Key Responsibilities
Essential Duties:
- Conducts thorough assessments of both acute and chronic clinical conditions. Documents physical evaluations and patient histories, analyzes patient condition trends, and formulates, documents, and executes a management plan based on the gathered data. This includes contributing to the nursing care plan and providing necessary counseling and education to patients and their families.
- Designs patient care strategies based on comprehensive knowledge of the specific patient demographic and protocols, while considering physiological and psychological challenges, as well as the patient's cultural background and support systems. Acts as an advocate for patients.
- Patient management encompasses the following:
- Creates orders for admissions, transfers, and discharges.
- Requests and interprets relevant laboratory and diagnostic tests.
- Prescribes appropriate medications and treatments.
- Refers patients to specialists as needed, including but not limited to dermatology, neurology, and psychiatry.
- Maintains detailed progress notes and summaries.
- May perform certain invasive procedures independently upon completion of required competency documentation.
- Engages in patient care rounds and discussions, communicating management strategies to the care team. Collaborates with multidisciplinary team members to ensure effective patient management.
- Identifies urgent situations requiring immediate physician intervention and initiates life-saving measures when necessary.
- Utilizes advanced communication skills to resolve complex issues and enhance patient service delivery.
Additional Responsibilities:
- Works with multidisciplinary team members to assess and improve healthcare delivery systems, implementing new practices as necessary.
- Participates in professional development activities that promote personal and professional growth.
- Collaborates with healthcare providers to facilitate patient transitions to lower levels of care when medically appropriate.
- Introduces self to patients and families, clarifying the role of the nurse practitioner.
- Facilitates family conferences to discuss treatment objectives, optimize resource use, and provide education.
- Fosters collaborative relationships to empower patients and families in making informed care decisions.
- Coordinates discharge processes to ensure appropriate care levels and utilizes preferred providers for additional services.
- Prescribes medications in accordance with state regulations.
- Performs other duties as assigned by management.
Qualifications:
- Demonstrated success in achieving clinical performance metrics in previous roles.
- Strong critical thinking and problem-solving abilities.
- Excellent communication and interpersonal skills.
- Effective time management skills, with the ability to perform well under pressure.
- Empathetic and caring demeanor with strong listening capabilities.
- Detail-oriented with a focus on patient care documentation and follow-up.
- Basic computer proficiency and familiarity with Microsoft Office Suite.
- This role may involve local travel.
Education and Experience Requirements:
- Advanced Nursing Practice (ARNP) or equivalent advanced nursing degree required.
- Current Nurse Practitioner Certification in the relevant state required.
- Board certification by AANP or ANCC preferred, depending on state regulations.
- Current DEA registration for controlled substances may be required based on state laws.
- A minimum of one year of clinical experience preferred, but strong new graduates will be considered.
- Basic Life Support (BLS) certification from an accredited organization required within the first 90 days of employment.
