Job Summary:
We are seeking a compassionate and detail-oriented Registered Nurse to join our Hospice team as a Case Manager. As a key member of our interdisciplinary team, you will be responsible for planning, organizing, and directing hospice care for our patients and their families.
Responsibilities:
- Assess patient needs and develop personalized care plans in collaboration with the clinical team.
- Coordinate and manage patient care services, ensuring timely and effective delivery of care.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
- Participate in interdisciplinary team meetings to discuss patient progress and develop care plans.
- Monitor patient outcomes and make adjustments to care plans as needed.
Requirements:
- Current unencumbered RN license.
- Minimum one year of experience as an RN in a medical-surgical or acute care setting.
- Computer proficiency in typing and various programs, including EMR.
- CPR certification (if required by state).
