Job Summary:
A successful Transfer Coordinator IV plays a vital role in facilitating the admission or transfer of patients from Strong Memorial Hospital Emergency Room to UR Medicine affiliate hospitals. This includes interviewing and consenting patients for transfer, answering patient and family questions regarding transfer to affiliate locations, and completing appropriate forms for transfer.

About the Role:
This is a full-time position that requires excellent communication skills, attention to detail, and the ability to work effectively in a fast-paced environment. The ideal candidate will have a Bachelor's degree in a related field and 3 years of experience in customer service, registration, data entry, or healthcare. They must be proficient in using electronic health records (EHRs) and have strong computer skills. The salary range for this position is $20.31 - $27.42 per hour.

Duties and Responsibilities:
- Coordinate transfers between Strong Memorial Hospital and affiliate locations, including arranging transportation and liaising with nursing staff.
- Review patients for transfer based on geographic proximity to affiliate hospitals and necessary services.
- Document transfers accurately and timely in the EHR system and update patient encounters for affiliate locations.
- Answer incoming calls for service requests and enter accurate data in the EMR system for RN follow-up.
- Collaborate with healthcare teams to facilitate communication regarding transfers and updates department staff regarding transportation.

Requirements:
- Bachelor's degree in a related field required.
- 3 years of experience in customer service, registration, data entry, or healthcare required.
- Proficient in using EHRs and strong computer skills required.
- Excellent written and verbal communication skills, as well as excellent customer service skills preferred.
