**Job Description:** Community Resource Specialist

VillageCareMAX seeks a dedicated professional to serve as the primary liaison between members and the organization's call center. This role plays a vital part in ensuring seamless access to essential services, resolving issues, and facilitating collaboration among members and the Interdisciplinary Care Team.

Main Responsibilities:
  • Responding to incoming phone calls from members and providers.
  • Guiding members on accessing covered and non-covered services in alignment with their care plans.
  • Entering authorization data under the supervision of the Care Management Supervisor.
  • Coordinating appointments with healthcare professionals and other service providers.
  • Supporting members in selecting participating providers to enhance care quality and outcomes.
Required Qualifications:
  • At least 3 years of experience in a related healthcare setting or call center environment.
  • High school diploma required; associate's degree preferred.
  • Proficiency in Cantonese and Mandarin languages.
Estimated Annual Compensation:

$65,000 - $80,000
