Job Title: Physical Therapist Home Health

We are seeking a skilled Physical Therapist to join our Home Health team at Alternate Solutions Health Network LLC. As a Physical Therapist, you will play a vital role in providing high-quality patient care and contributing to the growth and development of our team.

Job Summary:

The Physical Therapist will be responsible for providing skilled physical therapy services to patients in their homes, as ordered by the attending physician. This includes observing, recording, and reporting patient reactions, changes, and progress to the attending physician and any appropriate agency staff.

Key Responsibilities:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Observe, record, and report patient reactions, changes, and progress to the attending physician and any appropriate agency staff.
  • Counsel patients and families on disease/injury management and proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
  • Supervise therapy services provided by PTAs and nurse assistants, as needed.
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Attend in-service training and mandatory agency meetings.
Requirements:
