We are seeking a skilled Physical Therapist to join our team at Alternate Solutions Health Network LLC. As a Physical Therapist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

About the Role:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Observe, record, and report patient reactions, changes, and progress to the attending physician and other relevant staff members.
  • Develop and implement individualized plans of care to help patients achieve their goals and improve their overall health and well-being.
  • Collaborate with other healthcare professionals to ensure comprehensive and coordinated care.
  • Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with treatment plans.
Requirements:
  • Minimum of one year of experience as a Physical Therapist in an acute setting.
  • Strong communication and interpersonal skills.
