Key Responsibilities:

  • Efficiently initiate care for new clients by performing initial evaluations and crafting individualized care strategies.
  • Deliver direct patient assistance, which includes support with daily tasks such as bathing, grooming, dressing, and mobility.
  • Oversee and guide a team of caregivers, ensuring compliance with care plans and the provision of high-quality services.
  • Uphold a safe and hygienic environment for patients, strictly following infection control guidelines.
  • Offer emotional support to patients and their families, creating a nurturing and compassionate setting.
  • Facilitate effective communication with healthcare professionals, family members, and other team members.
  • Work collaboratively with fellow healthcare professionals to guarantee comprehensive and holistic patient care.
  • Meet clients for initial assessments, evaluate their needs, and develop personalized care plans to assist with their Activities of Daily Living (ADL).
  • Provide direct in-home caregiving, aiding with personal care, medication management, mobility, and other ADLs as necessary.
  • Mentor and support other caregivers, enhancing their skills and capabilities.
  • Collaborate with the care team and other stakeholders to ensure effective communication and coordination of care.
  • Availability for weekend on-call rotation is required.
  • On-call scheduling responsibilities will be part of the role.
  • Lead Caregivers or Lead CNAs will be responsible for covering any call-outs during designated hours and initiating care for new clients as needed.
