Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Harmony Trace, an assisted living and memory care community in Hilliard, Ohio.

About the Role:

As a Licensed Practical Nurse, you will be responsible for providing high-quality nursing care to our residents, working closely with our interdisciplinary team to ensure the best possible outcomes.

Key Responsibilities:
  • Supervise, provide, and direct nursing care in compliance with facility policies and procedures.
  • Make observations, report, and record observations appropriately, forming nursing judgments and carrying out nursing actions.
  • Follow established standards of nursing practices, assuming responsibility for compliance with federal, state, and local regulations.
  • Perform daily rounds, develop, and implement accurate comprehensive care plans based on each resident's needs.
  • Follow staffing guidelines, make nursing assignments, and receive and give accurate reports on resident conditions to provide continuity of care.
  • Monitor diets, supervise meal trays and snacks, and assist in passing meal trays, feeding, and passing snacks.
  • Follow facility admissions procedures, communicate admission to other departments, staff, physicians, and other service providers.
  • Supervise employees, ensuring they follow the chain of command in a manner consistent with personnel policies.
  • Assign nursing assistants, ensure efficient completion of assignments, supervise, document, and correct poor performance.
  • Participate in performance evaluation processes, provide counseling, coaching, and training, and present performance evaluations to staff.
  • Communicate professionally and effectively, following written and verbal directions, providing exemplary customer service, and reporting changes in physical or mental conditions.
  • Follow all company, facility, and state policies and procedures, attending staff meetings, in-services, inter-disciplinary team, and committee meetings as assigned.
  • Exercise keen observations of noise, odors, cleanliness, etc., cooperating with other staff members to ensure quality environmental conditions for residents and staff.
  • Document resident information timely, accurately, and confidentially according to procedures, verifying change-over MARS/TARS are correctly updated and necessary changes are made to new MAR/TAR by the 1st of each month.
  • Adhere to guidelines as outlined in the Resident Rights documentation, knowing resident rights and helping residents exercise and/or protect their rights.
  • Report resident complaints to the supervisor in charge, maintaining confidentiality, and adhering to all policies and procedures as outlined under the Safety & Compliance policies and procedures.
  • Ensure residents are safe, using protective equipment, following infection control protocol standards, policies, and procedures, practicing universal precautions, and life safety measures.
