We are seeking a skilled Physical Therapist to join our team at HexaQuEST Health, Inc. in a Rehabilitation Therapy role.

Key Responsibilities:
  • Provide high-quality physical therapy services to patients in a skilled nursing facility setting.
  • Conduct thorough assessments and develop individualized treatment plans to meet patient needs.
  • Collaborate with healthcare teams to ensure seamless care coordination.
  • Document patient progress and maintain accurate records.
Requirements:
  • Current state license to practice as a Physical Therapist.
  • Less than 1 year of experience in a skilled nursing facility setting.
  • Ability to work Monday through Friday shifts.
  • COVID-19 vaccination or medical/religious exemption required.
