Job Description

Job Summary: We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team at Community Consolidated School District 89. The successful candidate will work with students with unique medical needs, providing medical procedures and supporting the educational team in providing services to students.

Responsibilities:

  1. Develop and Implement Health Plans: Collaborate with the Certified School Nurse and Building Nurse to develop individualized health plans for students with medical needs.
  2. Provide Medical Procedures: Perform medical procedures such as blood glucose monitoring, catheterizations, gastrostomy tube care, and tracheostomy care as necessary.
  3. Monitor and Assist Students: Monitor and assist students in daily routines and help them follow individual daily schedules.
  4. Document Health Status: Document procedures and health status of students on a daily log.
  5. Communicate with Team Members: Communicate changes in health status of students to the Certified School Nurse/Building Nurse and Teacher.
  6. Handle Medical Emergencies: Handle medical emergencies appropriately and classroom problems quickly and efficiently.
  7. Provide Instruction: Provide instruction to students individually and/or in small groups following the Teacher's lesson plans and with Teacher direction.
  8. Accompany and Assist Students: Accompany, assist, and monitor students as needed in all other areas of the school program outside of the classroom.
  9. Implement Behavior Management Plans: Implement established classroom and individual behavior management plans.
  10. Professionalism and Confidentiality: Demonstrate a high level of professionalism with the ability to maintain confidentiality and personal flexibility in all areas.
  11. Meetings and Attendance: Participate in/attend building and district meetings, as required.

Qualifications:

  1. Registered Nurse or LPN Licensure: Registered Nurse or LPN licensure in the State of Illinois.
  2. First Aid and CPR/AED Certification: Current First Aid and CPR/AED Certification.
  3. Age Requirement: At least 21 years of age.
  4. ISBE Paraprofessional License: ISBE Paraprofessional License (See attached for assistance as this is an easy process).
  5. Experience with Special Needs Students: Experience in working with special needs students.
