About the Role

We are seeking a highly skilled and compassionate Personal Service Coordinator to join our team at Clarvida - California. As a key member of our team, you will be responsible for coordinating the needs of each client and family in their caseload, ensuring that they receive the highest level of care and support.

Key Responsibilities
  • Conduct comprehensive evaluations of each client's life situation, problems, and needs, and develop a detailed plan of care with their participation and consent.
  • Provide professional support to clients by maintaining regular contact, including client home visits, to provide personal support, consultation, confrontation, and intervention as necessary.
  • Facilitate access to and provide community resource linkage, and manage the process to include monitoring the effectiveness of support services.
  • Maintain daily records of work activities, completing daily service progress notes and billing forms in compliance with Company and government standards.
  • Develop and maintain professional working relationships with agencies and service providers.
  • Meet monthly productivity goal requirements and provide exceptional customer care.
Requirements
  • Bachelor's degree from an accredited university in human services or a related field, or two years of related experience.
  • Valid driver's license and proof of automobile insurance.
  • Excellent documentation and organization skills, interpersonal skills, and good written and verbal communication skills.
  • Intermediate level PC skills required.
