We are seeking a skilled Physical Therapist to join our team at Gables Search Group. As a Physical Therapist, you will play a vital role in helping our residents achieve their physical therapy goals.
Job Summary:The Physical Therapist will evaluate and treat residents with physical disabilities or impairments, using a variety of therapeutic procedures to restore function and mobility. The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work effectively with a multidisciplinary team.
Responsibilities:- Evaluate residents' physical abilities and develop individualized treatment plans to improve function and mobility.
- Conduct and document thorough assessments of residents' physical condition, including home and vocational needs.
- Develop and implement treatment plans that are tailored to each resident's needs and goals.
- Provide instruction and supervision to assistants and other staff members as needed.
- Enter progress notes and maintain accurate department records.
- Revise treatment plans as needed to ensure optimal outcomes.
- Document discharge summaries and provide clear, concise information to residents and families.
- Orient residents to program services and treatment procedures.
- Work collaboratively with nursing staff, residents, and families to maximize residents' use of functional abilities.
- Instruct residents and families on exercise programs and therapeutic procedures to be continued at home.
- Assist in the referral process for additional services following discharge.
- Participate in rehabilitation team conferences and other department meetings as required.
- Stay current with continuing education opportunities to maintain clinical competency.
- Participate in quality improvement initiatives.
- Promote the programs and services of Gables Search Group through formal and informal interactions with the community.
- Adhere to facility policies and procedures and participate in facility quality improvement and safety programs.
- Perform other duties as assigned to provide quality care to residents.
- Bachelor's or Master's degree in Physical Therapy.
- Current license in the state of practice.
- Prefer supervision, training, and/or experience in a setting serving the same age/type of resident population served by this facility.
- Certification, if applicable, and demonstrated competency in identified areas of specialization.
- Excellent oral and written communication and interpersonal skills.
- Demonstrated competency in applying the principles, methods, materials, and equipment used in physical therapy.
- Ability to assess resident needs and develop and implement a comprehensive plan of care.
- Knowledge of accreditation standards and compliance requirements.
The Physical Therapist will be required to perform the following physical activities:
- Standing for prolonged periods of time.
- Pushing and lifting objects up to 50 pounds independently.
- May be required to lift over 50 pounds with assistance of another person or a mechanical lift.
- Stooping, bending, and reaching.
- Balancing on narrow, slippery, or moving surfaces.
