About the Role

We are looking for an experienced Physical Therapist to provide high-quality patient care in a senior living community setting. As an outpatient Physical Therapist, you will be responsible for making home visits to clients, assessing their needs, and developing personalized care plans.

The ideal candidate will have a strong background in physical therapy, excellent communication skills, and the ability to work independently. If you are passionate about delivering exceptional patient care and have a desire to grow professionally, we encourage you to apply.

Responsibilities
  • Making home visits to clients in designated senior living communities
  • Performing diagnostic tests and measurements to assess client needs
  • Developing and implementing individualized care plans
  • Continually assessing and revising client care plans as needed
  • Educating clients and their families on therapy services and activities necessary for independent living
Requirements
  • A current VA Physical Therapist license
  • Outpatient experience preferred but not required
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
  • Ability to work independently and manage time effectively
  • Dynamic and strong interpersonal, organizational, and problem-solving skills
