To provide high-quality patient care, under the supervision of a physician, in a dynamic and collaborative environment. This role is administratively supervised by the Clinical Coordinator or designee.
Key Responsibilities:- Conduct thorough assessments of patients' clinical, emotional, and educational health needs.
- Collaborate with physicians to develop and implement plans of care, ensuring timely follow-up and communication.
- Provide ongoing emotional support to patients and their families, both in formal groups and during individual encounters.
- Participate in research activities and contribute to the development of protocols relevant to department functioning.
- Facilitate referrals to community agencies and participate in patient education activities.
- Collaborate with physicians and other clinical staff to maintain continuity of care and ensure high-quality patient outcomes.
- Minimum one (1) year of primary care experience required.
- Masters degree in Nursing required, with graduation from a NLN accredited NP program.
- Registered Nurse License (Virginia) within 6 months of hire, and Registered Nurse License (Maryland) within 6 months of hire, or Compact License: Registered Nurse within 6 months of hire.
- Nurse Practitioner License (Virginia) within 6 months of hire, Certified Nurse Practitioner Certificate (District of Columbia) within 6 months of hire, Nurse Practitioner Certificate (Maryland) within 6 months of hire, Registered Nurse License (District of Columbia) within 6 months of hire, and Nurse Practitioner National Certificate.
- Two (2) years of primary care experience preferred.
