Primary Care Physician

As a Primary Care Physician, you will be part of a person-centered caring culture that values delivering compassionate, quality healthcare in partnership with patients, community organizations, and staff.
Responsibilities- Provide comprehensive adult primary care services, including preventive and sick care, for adult patients of local communities.
- Coordinate patients' specialty care referrals utilizing all available resources and follow up to ensure continuity of care.
- Ensure patient/provider continuity and access to timely and appropriate levels of care.
- Supervise mid-level providers, as required by the practice, and ensure they meet the criteria for credentialing and privileging in the Department of Medicine.
- Participate in the H+H Antibiotic Stewardship program by following prescribing guidelines, educating patients and family members about the risks of antibiotic overuse, and helping provide education to non-clinical staff on this subject.
- Maintain a work schedule, including evenings and weekends, that ensures availability and continuity of care.
- Meet all compliance requirements, including health and safety, infection control, patient confidentiality, sexual harassment, domestic violence, and customer training.
- Graduation from an approved Medical School.
- Licensed to practice in New York State.
- Completion of an approved residency in the specialty and Board Certified or Eligible (if Eligible, must obtain certification within five years of eligibility).
- Meet the criteria for credentialing and privileging in the Department of Medicine as defined by the Medical Staff Bylaws of both the Gotham Health Care and Woodhull Medical and Mental Health Center.
