Key Responsibilities- Patient Assessment and Care Planning: Conduct thorough assessments of patients' physical, emotional, and social needs, and develop comprehensive care plans to address these needs.
- Family Support and Education: Provide emotional support and education to patients and their families, helping them to understand their care options and make informed decisions.
- Interdisciplinary Collaboration: Work closely with our interdisciplinary team, including physicians, nurses, and other healthcare professionals, to ensure seamless care coordination and communication.
- Community Resource Referral: Identify and refer patients to community resources and services that can support their needs, such as home health care, adult day care, and counseling services.
- Documentation and Reporting: Maintain accurate and timely documentation of patient interactions, care plans, and outcomes, and provide regular reports to our team and stakeholders.
- Education and Experience: Bachelor's degree in Social Work or related field, with at least one year of experience in a healthcare setting.
