Job Summary

We're seeking a highly skilled Licensed Behavioral Health Specialist to join our team. As a key member of our care management team, you'll work closely with patients to provide behavioral health support and resources. Your expertise will help patients manage their mental health conditions, improve their health literacy, and achieve their wellness goals.

Responsibilities
  • Utilize the Targeted Enrollment List (TEL) to make outreach with patients in need of care.
  • Review Complex Behavioral Health cases and Comprehensive Health Assessments (CHA) as required by managed care plans.
  • Provide brief behavioral health therapeutic interventions to improve patients' ability to manage their own health.
  • Handle brief crisis interventions and make warm hand-offs to local crisis resources as needed.
  • Educate patients and families about chronic medical and behavioral health conditions to improve health literacy.
  • Provide formal and informal training and support for patients on behavioral health conditions, including treatments.
  • Assist with the coordination of medical and behavioral health access issues with primary care physicians, specialists, and ancillary services.
  • Track medical and behavioral health outcome measures in a patient registry.
  • Coordinate with psychiatric hospitals to ensure smooth transitions of care.
  • Participate in weekly systematic and ad hoc case reviews to collaborate with the care management team on patient care issues.
  • Work with patients to identify health and wellness goals to incorporate into their shared care plan with providers.
  • Review patient community health assessments (CHAs) upon completion by another care team member to ensure accuracy.
  • Maintain engagement with patients using evidence-based approaches that promote collaboration between the patient and their medical/behavioral team.
Qualifications

To be successful in this role, you'll need:

  • Two years of experience in healthcare.
  • A valid LCSW, LMFT, PhD/PsyD, or LPCC license.
  • Experience with community outreach, behavioral health, and working with high-risk populations is preferred.
  • Bilingual skills (English/Spanish) are preferred.
What We Offer

We offer a competitive salary range of $92,500-$113,500 per year, depending on experience. This position is also eligible for a $5,000 signing bonus.
