Job Summary

CVS Health is seeking a skilled Nurse Case Manager to join our team. As a key member of our healthcare team, you will be responsible for assessing, planning, implementing, and coordinating case management activities with members to evaluate their medical needs and facilitate their overall wellness.

Key Responsibilities

  • Telephonically and/or face-to-face assess, plan, implement, and coordinate case management activities with members.
  • Evaluate the medical needs of members to facilitate their overall wellness.
  • Develop a proactive course of action to address issues presented to enhance short and long-term outcomes and opportunities to enhance a member's overall wellness through integration.
  • Services strategies, policies, and programs are comprised of network management and clinical coverage policies.

Requirements

  • 3+ years of clinical practice experience.
  • Minimum 2+ years of Case Management in a Managed Care environment.
  • Active unrestricted NC Compact RN Licensure.

Preferred Qualifications

  • Previous Case Management or Managed Care experience.
  • Previous experience with Home Health.

Work Environment

This role is performed face-to-face/field-based and requires those in the position to travel at least 50% of the time outside of the home/office to various member homes, various facilities (such as nursing homes, rehab centers, etc.), or community meetings on a defined schedule, adhering to the defined metrics, as required by the business.
