Key Responsibilities:
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education.
- Conduct thorough assessments of patients' physical and emotional needs, developing individualized care plans to meet their unique requirements.
- Collaborate with patients, families, and healthcare providers to ensure seamless care coordination and communication.
- Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
- Greater than two years of home or public health nursing experience preferred.
- Previous home care experience desired but not required; we are willing to train and develop a candidate who demonstrates a willingness and desire to learn home health nursing.
- Graduate nurses are considered for our Community Health Nurse I level.
Benefits:
- $10,000 sign-on bonus for full-time external applicants, prorated for part-time roles.
- Mileage reimbursed at Federal Gas Rates.
On-Call:
Not Required.
