Job Title: Primary Care Nurse

At Signature Health, we are seeking a highly skilled and compassionate Primary Care Nurse to join our team. As a Primary Care Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.

Job Summary:

The Primary Care Nurse will be responsible for providing comprehensive nursing care to patients in a primary care setting. This includes assessing patient needs, developing and implementing individualized care plans, and collaborating with healthcare providers to ensure seamless patient care.

Key Responsibilities:
  • Assess patient needs and develop individualized care plans
  • Provide comprehensive nursing care to patients in a primary care setting
  • Collaborate with healthcare providers to ensure seamless patient care
  • Conduct thorough and accurate reviews of patient medications and update medication lists
  • Perform procedures and specimen collection, including phlebotomy as needed
  • Complete appropriate documentation in the medical record to comply with state, federal, and departmental policies
  • Coordinate excellent patient care with staff of outside healthcare facilities or emergency rooms when necessary
  • Assist in coordination of care for patients with other service lines within and outside Signature Health
Requirements:
  • Completion of an approved program in registered nursing required
  • Valid unencumbered RN license in the state of Ohio required
  • 1+ years' experience in a primary care, medical-surgical or acute care setting required
  • American Heart Association (AHA) Basic Life Support (BLS) certification required
  • Bachelors of Science in Nursing (BSN) preferred
  • Bi-lingual in English and Spanish preferred
Working Conditions:

The work environment is typically interior/office/clinical, with flexibility to work evenings and extended hours as needed. The physical environment requires occasional lifting and carrying items weighing up to 15 pounds, unassisted. The employee must be able to stand for long periods of time and perform frequent bending, reaching, and repetitive hand movements.

Sufficient dexterity to operate a PC and other office equipment is required. Possible exposure to blood-borne pathogens while performing job duties may occur. The employee must be able to work in a fast-paced environment and maintain a high level of professionalism and confidentiality.
