Key Responsibilities:
- Oversee, complete, and assess resident evaluations and care plans in compliance with relevant state and federal regulations, ensuring residents achieve optimal functioning.
- Assume responsibility for coordinating the MDS, CAAS, and care planning processes for residents upon their admission.
- Conduct a thorough review at the start of each month for all Medicare, Managed Care, and outpatient therapies.
- Monitor and report any changes in residents' physical and emotional well-being to the Unit Nurse.
- Manage and update the 672/802 forms on a weekly basis as required.
- Establish care plan timelines within seven days of completing the MDS or during quarterly reviews, adhering to state and federal guidelines.
- Ensure nursing documentation aligns with the care plan during the provision of nursing care or treatment.
- Record information gathered during the assessment process accurately.
- Set Assessment Reference Dates (ARD) for residents and communicate these with the interdisciplinary team.
Qualifications:
- Graduate from an accredited LPN/RN program.
- Valid LPN/RN license in Illinois.
- Minimum of two years of experience in MDS.
- Recent clinical experience and specialized skills in geriatrics.
- Flexible availability, including evenings, weekends, and holidays as necessary.
- Passion for working with the senior community.
