Key Responsibilities- Provide comprehensive, evidence-based care to patients with acute and chronic illnesses, focusing on prevention, early intervention, and coordinated care.
- Lead a care team to ensure seamless patient care, including care promoters, care facilitators, and care coordinators.
- Engage with hospitalists, case managers, and other healthcare professionals to ensure continuity of care.
- Participate in quality improvement initiatives, risk management programs, and clinical meetings to drive excellence in patient care.
- Develop and maintain strong relationships with patients, families, and caregivers to build trust and ensure patient satisfaction.
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.
- Current, active State Medical License for the state(s) in which you'll be working.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty preferred.
- Excellent communication, leadership, and interpersonal skills.
- Ability to work independently and collaboratively as part of a multidisciplinary team.
