Job Summary- The Multidisciplinary Social Service Specialist provides comprehensive medical social casework to diverse patient populations, addressing their psychosocial needs to ensure optimal functioning.
- This role involves direct care services, consultation, education, and community liaison, while managing high-risk referrals.
- Assist patients and families in meeting their psychosocial/continuum of care needs.
- Provide direct care services, including assessment, planning, implementation, and evaluation of patient care plans.
- Collaborate with healthcare professionals to ensure seamless coordination of care.
- Manage high-risk referrals and provide consultation to other staff members as needed.
- Masters degree in Social Work (MSW) from an accredited institution.
- Current Social Work license in the state of South Dakota required. Current Social Work license in Iowa, Nebraska, Minnesota, or North Dakota may be required depending on the service area.
- Valid driver's license and insurability by Avera Health's automobile liability insurance carrier.
- Competitive hourly rate ranging from $28.75 to $43.50 per hour, dependent on experience.
- Paid Time Off (PTO) available on Day 1.
