Key Responsibilities:

  • Assist clients with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) as per the care plan developed by the Nurse Case Manager.
  • ADLs may include personal hygiene, dressing, grooming, mobility assistance, and toileting.
  • IADLs may involve meal preparation, light housekeeping, running errands, accompanying clients to appointments, and facilitating communication.
  • Perform additional tasks as assigned to ensure comprehensive care for clients.
