Job Summary

Tennessee Quality Care is seeking a skilled Nurse Care Specialist to provide high-quality patient care and support to our patients and their families. As a key member of our healthcare team, you will play a vital role in delivering exceptional patient outcomes and experiences.

Key Responsibilities
  • Direct Patient Care: Provide direct patient care as defined in the state Nurse Practice Act, ensuring the highest level of care and compassion for our patients.
  • Plan of Care Implementation: Implement plans of care initiated by registered nurses, working collaboratively to ensure seamless patient care.
  • Documentation and Communication: Provide accurate and timely documentation consistent with the plan of care, ensuring effective communication with patients, families, and healthcare teams.
  • Patient Education: Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
  • Home Health Services Coordination: Participate in coordination of home health services, identifying needs for other disciplines and reporting to registered nurses and Clinical Supervisors.
  • Equipment and Supply Management: Use and/or prepare equipment and supplies effectively and efficiently, adhering to aseptic technique.
  • Personal and Professional Growth: Participate in personal and professional growth and development, staying up-to-date with industry best practices and advancements.
  • Additional Duties: Perform other duties as assigned by registered nurses, ensuring a seamless patient care experience.
Requirements
  • Nursing Education: Graduate of an accredited practical nurse or vocational nursing program.
  • Nursing Experience: Three (3) years nursing experience, with community/home health or medical/surgical experience preferred.
  • Licensure and Certification: Currently licensed as a licensed practical nurse or licensed vocational nurse in the state, with valid CPR certification.
  • Transportation and Insurance: Must have reliable transportation, a current driver's license, and appropriate automobile insurance.
