Care Manager RN - Seasonal

**Job Summary**

We are seeking a skilled Care Manager RN to join our team at ShorePoint Health Port Charlotte. As a key member of our healthcare team, you will play a vital role in coordinating patient care and ensuring seamless transitions between healthcare services.

**Key Responsibilities:**

* Coordinate the overall interdisciplinary plan of care for patients, from admission to discharge
* Serve as a liaison between patients, families, and healthcare personnel to ensure timely and effective care
* Utilize experience and judgment to plan and accomplish goals, performing a variety of tasks with a high degree of creativity and latitude
* Typically reports to a head of a department/unit
* Requires a registered nurse with at least 2 years of clinical experience and an associate's degree, with a bachelor's degree preferred
* Familiarity with standard concepts, practices, and procedures within a particular field
