Clinical Lead for Mental Health Services

Join LifeBridge Health, a leading healthcare provider in Maryland, and take on the role of Clinical Lead for Mental Health Services. In this position, you will be responsible for providing clinical intervention to clients with Severe and Persistent Mental Illness as part of a multi-disciplinary team.
Job SummaryThe ACT team social worker provides critical support to patients by offering case management, transportation, individual/group/family therapy sessions, motivational interviewing, supportive therapy techniques, psychiatric rehabilitation skill building, and crisis intervention. The primary focus is on delivering these services in community settings such as homes, public areas, shelters, etc. in Baltimore City and County.
A key aspect of this role involves assessing patients' family/social support systems to identify strengths/barriers impacting mental health stability. Additionally, you will collaborate with other healthcare team members to coordinate patient care, develop comprehensive goal plans, and revise treatment plans as necessary.
Responsibilities- Provide individualized and family/Support System therapy as needed.
- Perform crisis assessments and interventions, implementing Emergency Petitions when necessary.
- Obtain client authorization for ACT services through the identified Maryland Managed Care Organization (MCO).
- Facilitate Integrated Dual Disorder Treatment (IDDT), specific groups, and other treatment groups deemed necessary by the team supervisor and Medical Director.
- Transport clients to and from office settings or appointments within the community.
- Offer general case management services, advocating for, procuring, and linking clients to community resources.
- Communicate client needs effectively to the rest of the staff.
- Promote client-centered treatment focused on the individual's needs.
- Promote good public relations for the program.
- Accept extra assignments as needed for overall program functioning.
- Must hold active licensure in Maryland as an LMSW/LGPC; LCSW-C/LCPC is preferred.
- Must have a valid MD Driver's License with a driving record containing no more than 2 points and ability to clear vehicle operation checks by the Fleet Safety team.
- At least one year of experience working with individuals with Severe and Persistent Mental Illness is required.
- Active BLS (CPR and First Aid) certification from the American Heart Association must be maintained.
