Senior Clinical Coordinator for Mental Health Services

Job Summary

This role requires a highly skilled and experienced mental health professional to lead a team in providing clinical interventions to individuals with severe and persistent mental illnesses. The ideal candidate will have a strong background in case management, individual and group therapy, crisis intervention, and psychiatric rehabilitation.

  • Assesses the patient's family/social support system and identifies strengths/barriers impacting mental health stability.
  • Coordinates patient care through consultation with healthcare team members.
  • Develops comprehensive goal plans in conjunction with individual and family members.
  • Develops, writes, implements, evaluates, and revises person-centered treatment plans in collaboration with the Community Treatment Team.
  • Provides individualized and family/support system therapy as needed.
  • Performs crisis assessments and interventions, implementing Emergency Petitions when necessary.
  • Obtains client authorization for ACT services using the identified Maryland Managed Care Organization (MCO).
  • Facilitates Integrated Dual Disorder Treatment (IDDT) groups and other treatment groups deemed necessary by the team supervisor and Medical Director.
  • Transports clients to office settings or appointments within the community.
  • Provides general case management services, including advocating for, procuring, and linking clients to community resources.
  • Communicates client needs to staff.
  • Ensures client treatment is client-centered and focused.
  • Promotes good public relations for the program.
  • Able to work autonomously in the community, visiting clients independently at their residence.

Requirements
  • Must hold active licensure in Maryland as an LMSW/LGPC, LCSW-C/LCPC.
  • Must have a valid MD Driver's License with a driving record with no more than 2 points and be cleared to operate vehicles by our Fleet Safety team.
  • Must have at least one year of experience working with individuals with Severe and Persistent Mental Illness.
  • Must have and maintain active BLS (CPR and First Aid) from the American Heart Association.
