About the Role

We are seeking a skilled and compassionate Registered Nurse to join our HomeHealth team as a Full Time Admissions RN. This role will support New Castle County, DE, specifically in Newark or Georgetown, Lewes, Rehoboth, working day shift hours, one weekend a month, and 2 holidays a year.

Key Responsibilities
  • Conduct thorough clinical assessments of patients at time of admission, documenting findings accurately.
  • Precept and mentor other nurses taking on admission duties, ensuring seamless transitions.
  • Participate in team meetings and interdisciplinary discussions to enhance patient care and practice.
  • Stay current in clinical practice, incorporating research and best practices into daily work.
Requirements
  • BSN required; ADN candidates must acquire BSN within three years of start date.
  • 2.5 years of recent skilled adult Home Health experience, including OASIS documentation.
  • OASIS proficiency required; certification must be acquired within 18 months.
  • IV proficient or willing to train, achieving proficiency within 3 months.
