Key Responsibilities- Harm Reduction and Substance Abuse Counseling
- Provide evidence-based, harm-reduction-focused substance abuse counseling to returning citizens, both within the jail setting and in the community.
- Develop and implement individualized treatment plans that address the unique needs of each client, focusing on reducing substance use and preventing relapse.
- Community-Based Behavioral Health Care
- Engage with clients in community and street-based settings to remove barriers to accessing care, including conducting outreach and follow-up visits.
- Coordinate with community resources and other healthcare providers to ensure a continuum of care that supports clients' successful reintegration.
- Recidivism Reduction Strategies
- Implement strategies to reduce recidivism by addressing the social determinants of health that contribute to substance use and criminal behavior.
- Provide ongoing support to clients as they transition from incarceration to community life, helping them navigate challenges and access necessary services.
- Client Advocacy and Support
- Advocate for clients within the healthcare and criminal justice systems, ensuring they receive the care and support they need.
- Assist clients in overcoming obstacles to treatment, such as transportation, housing, and access to medications.
- Collaborative Care and Communication
- Work closely with a multidisciplinary team, including case managers, social workers, and other healthcare providers, to deliver comprehensive care.
- Maintain clear and consistent communication with team members to ensure that all aspects of clients' care are coordinated and effective.
- Documentation and Reporting
- Accurately document all client interactions, treatment plans, and progress notes in accordance with legal and ethical standards.
- Prepare and submit regular reports on program outcomes and client progress to support program evaluation and improvement efforts.
- Licensure: Must hold a current New Mexico LSAA or LADAC license.
- Experience: Previous experience in substance abuse counseling, particularly within re-entry or community-based settings, is preferred.
- Skills: Strong understanding of harm reduction principles, excellent communication and advocacy skills, and the ability to work effectively in a multidisciplinary team.
- Commitment: A deep commitment to serving underserved populations and reducing healthcare disparities through equitable, compassionate care.
