Job Description

We are seeking a highly skilled and compassionate Substance Abuse Counselor to join our team at Hire Health. As a Counselor, you will play a vital role in providing comprehensive treatment services to individuals struggling with addiction.

Responsibilities
  • Conduct intakes, discharge planning, initial assessments, and initial treatment plans in a professional and timely manner.
  • Involve patients in the assessment, treatment planning, and intervention aspects of treatment.
  • Administer biopsychosocial assessments to evaluate patients' lives, including their addictions, medical and mental health history, socioeconomics, and legal status.
  • Develop and implement individualized treatment plans that effectively help patients meet their short- and long-term goals.
  • Monitor patients' progress and document any changes in their circumstances, identifying and addressing relapse indicators to promote relapse prevention interventions.
  • Comply with and implement Medical Q.A (Quality Assurance) and Clinical Risk Management Policies.
  • Administer screening tools to determine the severity of patients' mental/emotional status.
  • Review and ask about other practitioners' involvement in the treatment of high-risk conditions.
  • Flag high-risk patients to Medical Directors, Physician Extenders, and Regional Clinical Leadership (RCL) and highlight the conditions concerned.
  • Refer patients to Medical Directors, Physician Extenders, or RCLs based on the urgency.
  • Review physician notes and verify that they noted and addressed high-risk factors.
  • Advise other staff members of high-risk patients and their condition.
  • Provide professional counseling and referral services by addressing patients' clinical and special needs.
  • Provide individual and group counseling as clinically necessary to address problem issues identified in the Initial Assessment and Treatment Plans.
  • Utilize advanced counseling techniques such as Cognitive Behavioral Therapy and Motivational Interviewing.
  • Schedule regular counseling sessions with patients and document counseling entries.
  • Engage in Community Relations and Outreach efforts.
  • Attend all recommended training, including Physician Training of Counselors, RCL training, and case conferences with program physicians and physician assistants.
  • Collect research data when requested.
  • Observe the collection of patients' urine samples when required.
  • Attend team meetings and complete all training courses timely as required.
Requirements
  • Less than 2 year(s): Counseling experience.
Education
  • Associates or better in Human Services.
Licenses & Certifications
  • LCSW.
  • CDAC.
  • CADC.
