SUMMARY/OBJECTIVE

Provide assistance and support to individuals with intellectual and/or developmental disabilities, primarily in their residential settings, to promote health and wellness, foster essential life skills for autonomy, and facilitate meaningful community engagement. This role aims to enhance independence within their living environment, educate about local resources, encourage social connections, and create enjoyable experiences through various activities and events, empowering each individual to reach their personal objectives. The position may also involve responsibilities at a day support facility.

KEY RESPONSIBILITIES

  • Implement cognitive interventions and train individuals in essential personal care skills (such as toileting, bathing, grooming, dressing, and eating).
  • Exhibit professional communication with individuals served, their families, and support systems while maintaining appropriate boundaries.
  • Assist with personal care and activities of daily living (ADLs), ensuring hygiene and nutritional needs are fulfilled.
  • Provide training on utilizing community resources (transportation, shopping, social and recreational activities).
  • Monitor health status and assist with medication management, as applicable.
  • Identify and report any signs of abuse, neglect, or exploitation.
  • Ensure the safety and well-being of individuals through supervision.
  • Facilitate behavioral support as needed and train in adaptive behaviors for home and community settings.
  • Encourage active participation of individuals and their families in the service delivery process, keeping track of daily goals and documenting progress.
  • Assist individuals in navigating developmental transitions and connecting them with community resources.
  • Complete required documentation promptly and maintain accurate timekeeping.
  • Ensure personal vehicle is equipped with current insurance and registration.

QUALIFICATIONS

  • High School Diploma or GED.
  • Valid driver's license and reliable transportation.
  • Preferred: 1 year of experience working with individuals with disabilities.
  • Ability to interact with others courteously and judiciously.
  • Clean driving record.
  • Possession of appropriate safety equipment and first aid supplies in personal vehicle.
