Emergency Services Case Manager

Piedmont Community Services is seeking a skilled Emergency Services Case Manager to join our team.
Job Summary:
We are looking for a highly motivated and organized individual to assist individuals referred through Emergency Services or Same Day Access in accessing services. The successful candidate will have a strong understanding of human services and be able to work effectively with clients, families, and community agencies.
Key Responsibilities:
- Assist with inpatient bed searches and identify potential clients
- Assess needs and plan services to meet client goals
- Link clients to services and supports, and coordinate services with other providers
- Enhance community integration and make collateral contact as needed
- Monitor service delivery and advocate for clients in response to changing needs
Requirements:
A Bachelor's degree in a related field, along with two years' experience in human services or a related field, is preferred. Basic knowledge of Emergency Services and recovery principles is also preferred. A sufficient combination of related education and experience is required, and salary is commensurate with education and experience as it relates to the position.
