Hospice RN Case Manager

We are seeking a compassionate and skilled RN Case Manager to join our team at Commonwealth Hospice. As a Hospice RN Case Manager, you will play a vital role in providing end-of-life care to patients and their families.
About the RoleAs a Hospice RN Case Manager, you will be responsible for:
- Conducting initial nursing evaluations to determine eligibility for hospice services
- Identifying patients' physical, psychosocial, and environmental needs and re-assessing as needed
- Developing and implementing individualized plans of care
- Collaborating with patients, families, and healthcare teams to provide comprehensive care
To be successful in this role, you will need:
- One year of clinical experience
- Current RN licensure in the state of practice
- Current CPR certification
- Access to a dependable vehicle or public transportation
