Job Summary: Concordia Visiting Nurses is seeking a skilled Physical Therapist to join our team in South Hills. As a Physical Therapist, you will be responsible for evaluating, planning, and implementing physical therapy services to patients in their homes or current residences, as prescribed by the attending physician.
About the Role: The Physical Therapist will work as a member of an interdisciplinary rehabilitation team, providing high-quality care to patients in need. You will have the opportunity to work with a variety of patients, each with unique needs and goals.
Responsibilities:
- Evaluate patients to determine their physical therapy needs and develop individualized treatment plans
- Implement physical therapy services, including exercises, stretches, and other interventions, as prescribed by the attending physician
- Communicate with patients, families, and healthcare teams to ensure seamless care
- Document patient progress and treatment plans
Requirements:
- Graduate of a state-approved school of physical therapy accredited by the American Physical Therapy Association
- Minimum of 6 months acute care or rehabilitation center experience
- Licensed as a Physical Therapist in the Commonwealth of Pennsylvania
- Reliable, independent means of transportation to make home visits
